The reliable way up the columns is specificity about mechanism. Better communication is a wish, while a read-back requirement at a named point in a named handoff is a design, and only the second one can be evaluated, budgeted, or refused.

  1. Translate every criterion into an observable act

    Under each criterion heading, write the sentence that will satisfy it. A criterion about analyzing collaboration is satisfied by naming a transfer point and what died there, never by a paragraph on the value of teamwork. Draft against your translations, then check the finished paper against the guide itself.

  2. Find the moment the information stopped moving

    Walk the case forward until you reach the exact handover where a fact existed on one side and not on the other. That single point is your object of analysis. Everything before it is context, and everything after it is consequence.

  3. Separate what people knew from what they were permitted to act on

    These fail independently. A nurse who knew and could not order, a therapist who could act and was never told, and a physician with authority and no prompt are three different faults with three different remedies. Say which one you have before proposing anything.

  4. Match a framework to the failure type

    Information lost in transit points to structured handoff and closed-loop confirmation. Risk seen and unspoken points to the psychological safety and speak-up literature. Disputed authority points to scope of practice documents and the interprofessional competency statements. Let the failure choose, then apply that one model throughout.

  5. Report what the teamwork evidence can and cannot carry

    Much of this literature is single-site before-and-after work measured by self-reported climate surveys, and adherence to structured tools decays once observers leave. Say so, then argue for a design that survives the decay. A limitation you raise yourself is a criterion banked.

  6. Specify the fix so it could be staffed on Monday

    Name the trigger, the person who performs the step, where it is recorded, who is escalated to when it is skipped, and the count that would show it working. Then self-score each criterion and give any level below the top one more pass before you submit.

The five mistakes that cost Distinguished

  • Communication treated as an attitude. Recommending that colleagues talk more gives the criterion nothing it can score.
  • The framework named and abandoned. A model defined in the introduction and never applied to a fact reads as decoration.
  • Every discipline blamed equally. Spreading fault across the team is a way of avoiding the analysis rather than performing it.
  • A fix with no owner or trigger. If nobody is named and no moment is specified, the recommendation cannot be implemented or graded.
  • Teamwork evidence quoted at full strength. Reporting climate-survey findings as though they were trial results invites a comment on every claim afterward.
