How BUS-FPX4014 Assessment 3 is scored

The number that carries this deliverable is the gap between catching a defect at the bench and catching it at the customer, which is routinely an order of magnitude. Put prevention, appraisal, internal failure and external failure in one table so the reader can see that gap, and the recommendation almost writes itself.

Outline from the criteria, then get the defect data into one table
Headings from the guide first. Then put every defect into one table with a cause, a count and a period, because a Pareto ordering cannot be built from prose. If the case supplies categories, use theirs rather than inventing tidier ones.

Order the causes by weight and state the concentration
Twenty-seven wrong dimensions, sixteen finish blemishes, eleven hardware misfits and seven packaging damages out of 61 defects means the top two causes are 43 of 61, or 70 percent. Say that figure out loud. The whole value of the technique is that it tells you which two problems to work on and which two to leave alone for now.

Separate ordinary variation from a real signal
A process that produces a bad week has not necessarily changed. Before recommending anything, say whether the pattern you are looking at sits inside the normal spread of the process or outside it, and on what basis. Reacting to ordinary variation as though it were a signal makes processes worse, which is a point worth making explicitly in the paper.

Build the cost of quality across all four categories
Prevention 6,200 dollars, appraisal 9,400, internal failure 28,700 and external failure 61,500 comes to 105,800 dollars, of which external failure is 58 percent. Four categories in one table, totalled, with percentages. A paper that costs only the scrap has costed the cheapest failure and missed the expensive one.

Justify prevention against the failure it displaces
Recommend the spend and show the trade. A fixture and a first-piece check costing 4,800 dollars a year that removes most of the dimension faults is arguing against the share of that 90,200 dollars in failure cost those faults produce. State the assumption about how much it removes rather than implying it removes everything.

Adapt the tool to the setting, then self-score
Lean and Six Sigma were developed in high-volume repetitive manufacturing, and applying their tools unchanged to low-volume high-variety work produces the appearance of rigour and very little improvement. Name the setting, say what you adapted, then mark your draft against each criterion and rewrite anything short of the top level before submitting early in the week.

Trellis Millwork logged 61 defects across 340 cabinet runs last year, and putting a figure against each of the four categories gives prevention at 6,200 dollars, appraisal at 9,400, internal failure at 28,700 and external failure at 61,500, a total cost of quality of 105,800 dollars.1 External failure alone is 58 percent of that, because a dimension fault found by the installer means a return visit, a remake and a site crew standing idle, while the same fault found at the bench means an hour of rework.2 The shop currently spends four dollars on finding defects for every dollar it spends on preventing them, and 43 of the 61 defects came from the two causes a first-piece dimensional check would catch.3

The five mistakes that cost Distinguished
- Defects listed without ordering. A table with no ranking gives no guidance about where to work, and the technique exists to produce exactly that guidance.
- Only scrap costed. Internal failure is the easiest category to measure and the least expensive one, and stopping there misses the argument.
- Ordinary variation treated as a signal. Reacting to normal spread as though the process had changed introduces new problems and reads as method misapplied.
- Prevention recommended with no trade shown. A spend has to be argued against the failure cost it displaces, with the assumption about how much it removes stated.
- Manufacturing tools imported unchanged. High-volume techniques applied to low-volume variety work look rigorous and change nothing, and the criteria reward naming the adaptation.
