Record decisions, not atmosphere
Write what was agreed and by whom: who supplies which report, which committee you may observe, what must be de-identified, how many hours per week the preceptor can supervise directly, and what happens if the site withdraws access. Delete anything that only conveys that the call went well. A reader should be able to enforce this document six weeks from now without having attended the call.

The five mistakes that cost Distinguished
- Objectives that cannot be evidenced. Gaining exposure to executive practice is not an objective, it is a hope, and the criterion asking for measurable outcomes has nothing to score.
- A preceptor without reach. If your objectives need position control data and committee access, a preceptor below director level will not be able to supply either, and the practicum stalls in week four.
- Hours planned as a total instead of a schedule. A single number for the whole course hides the weeks in which nothing was arranged. Distribute the hours and the plan starts predicting its own failure points.
- A problem with no baseline. Low morale and poor retention are categories. A vacancy rate, a time to fill, or a first-year turnover figure is a problem you can close.
- Loose confidentiality terms. Write the de-identification rule into the record. A practicum document that anticipates naming units and individuals invites a faculty correction before you have written a word of analysis.
